Free Diabetes Education. Expanding to Chronic Illness Care

Trusted, no-cost health education for rural communities — starting with diabetes, expanding to chronic illness care, advancing rural health equity.

Speaker

What It Is

SozoRock Access Day is a non-clinical, public health education initiative for rural and underserved communities. We start with diabetes prevention and management, expanding into chronic illness care to strengthen rural health equity.

Two Coordinated Tracks

  • Resident Education
Practical, plain-language health sessions led by licensed professionals, with take-home resources designed for rural access.
  • Stakeholder Roundtable
County leaders, providers, nonprofits, and universities align on data, resources, and partnerships to improve community health infrastructure.

Funded entirely by SozoRock Health, Access Day involves no platform promotion, no billing, and no insurance requirements.

Speaker

Why It Matters

Rural communities face rising diabetes rates and limited access to consistent health education. Access Day delivers licensed professionals, evidence-based strategies, and ready-to-use resources directly where they are needed most — at no cost and with no barriers.

Impact

  • Residents leave with tools and knowledge they can use immediately.
  • Stakeholders leave with actionable insights and new partnerships to extend care access beyond the event.

This is more than a single day — it's the starting point for a consistent, trusted health education presence that builds long-term rural health equity.

Stakeholder Roundtable

The Stakeholder Roundtable unites county leaders, healthcare providers, nonprofits, and universities to advance rural health equity. Participants collaborate to share data, align resources, and form partnerships that extend access to trusted, no-cost health education.
