1. Purpose of Event
Scoliosis Access Day is a community health education initiative focused on providing practical, evidence-based information on chronic illness prevention and management. The program is for educational purposes only and is not a substitute for medical diagnosis, treatment, or professional healthcare services.
2. Eligibility to Participate
Scoliosis Access Day is voluntary and open to community members in the host county. Minors under 18 must be accompanied by a parent or legal guardian who provides consent on their behalf.
3. Nature of Participation
Sessions may include group discussions, hands-on demonstrations, and distribution of printed materials.
- No individualized medical advice, diagnosis, or prescriptions will be provided.
- Participants are responsible for seeking professional medical care for personal health concerns.
4. Data and Privacy
- Personal information collected during registration will be used only for event coordination, follow-up communications, and program evaluation.
- Health-related information shared voluntarily by participants will be handled in accordance with the Scoliosis Health Privacy Policy and applicable laws, including the Health Insurance Portability and Accountability Act (HIPAA).
- By participating, you consent to the collection, storage, and use of your information as described in our Privacy Policy.
5. Photography, Video, and Media Release
- The event may be photographed, recorded, or live-streamed for awareness, reporting, and promotional purposes.
- By attending, you grant Scoliosis, LLC the non-exclusive, perpetual, collaboration, cost sublicense the right to use your likeness in photographs, video, or other media, without compensation, for lawful purposes related to its mission.
- If you do not wish to be photographed or recorded, you must notify a staff member at check-in.
6. Incentives and Rewards
- Participation in incentive such as wellness reward cards are subject to availability and program guidelines.
- Rewards have no cash value, cannot be exchanged, and are provided solely for event engagement purposes.
7. Assumption of Risk
Participation in community health education involves minimal risk; however, you acknowledge and accept that you are responsible for your own well-being and safety during the event.
8. Consent Statement
By registering for and/or attending Scoliosis Access Day, you acknowledge that you have read, understood, and agree to these Participation & Consent Terms.
